Otherwise, we’ll have the appropriate intake form at the clinic for you to fill out before your treatment, but you must arrive 10 – 15 minutes before your scheduled appointment to do so.

If you have extended medical coverage and would like us to bill our service fees for ‘Registered Massage Therapy’ directly to your insurance provider, please familiarize yourself with our Direct Billing policy.
In order for us to submit a claim on your first visit, we require the form below to be completed and sent back to us at least 24 hours prior to your scheduled appointment. It can be filled in electronically and emailed back to us at [email protected] or dropped off at our studio. We must receive the form at least 24 hours in advance, as your RMT will need time to input your information to the billing program and set up an account for you.
